Does Wegovy give you heartburn? What to do if reflux starts after treatment

Heartburn can sometimes appear or feel worse while taking Wegovy. For most people, this is linked to how the medication slows digestion and is usually temporary, but it can still be uncomfortable and worrying when it happens.

In this blog, we’ll explain why Wegovy can trigger heartburn, when symptoms are most likely to occur, what you can do to ease them and when it’s important to seek medical advice.

Wegovy and heartburn

Wegovy (semaglutide) can cause or worsen heartburn in some patients. While heartburn is not among the most common side effects, reflux-type symptoms have been recognised in clinical trials. Gastro-oesophageal reflux disease (GORD) was reported in around 1.7–1.8% of people taking Wegovy compared with about 0.7–0.8% on placebo, and more significant gastrointestinal side effects were seen in a small proportion of users. This means most people do not experience severe reflux however it is a known and documented possibility.

Why can Wegovy trigger reflux symptoms?

Wegovy works by slowing how quickly the stomach empties food into the gut. This can leave food sitting in the stomach for longer which increases pressure inside the stomach and can make it easier for acid to travel back up into the oesophagus (the tube between the mouth and stomach), leading to heartburn. Nausea and occasional vomiting can also make reflux worse, particularly during dose increases or when the body is still adjusting. In rare cases, repeated vomiting has been linked with more severe inflammation of the oesophagus, highlighting why these symptoms should be monitored rather than ignored.

When is heartburn most likely to happen on Wegovy?

Heartburn and reflux symptoms are most commonly noticed in the first few weeks after starting Wegovy or following a dose increase, rather than later in long-term stable treatment. This pattern is well recognised in clinical trials where digestive side effects tend to cluster during initiation and dose escalation phases. Symptoms often improve as the body adapts but if heartburn is persistent, worsening or interfering with eating and hydration, it should be reviewed so the treatment plan can be adjusted safely.

Practical ways to ease Wegovy heartburn symptoms

If you develop heartburn while taking Wegovy, there are several simple steps that may help reduce symptoms before they become more disruptive:

  • Eat smaller meals and slow down your eating to avoid overfilling the stomach.
  • Avoid large, rich or high-fat meals, which can make reflux worse.
  • Try not to lie down soon after eating and aim to have your last meal earlier in the evening.
  • Keep a note of any trigger foods or drinks that seem to worsen your heartburn, such as spicy foods, caffeine or alcohol.
  • If needed, speak to a pharmacist about suitable over-the-counter options for short-term relief.
  • Do not stop or reduce your Wegovy dose without speaking to your prescriber first. With the right adjustments, many people find these symptoms improve as their body adapts to treatment.

When should I seek medical attention?

Mild heartburn can often be managed with simple lifestyle changes, but it’s important to know when symptoms should be reviewed. If you experience ongoing or returning heartburn while taking Wegovy, especially if it is not improving with self-care, you should contact our clinical team at Phlo Clinic for advice.

You should contact our clinical team if you have:

  • Heartburn or reflux that keeps coming back or is not improving.
  • Ongoing nausea, bloating or indigestion affecting your day-to-day life.
  • Symptoms that worsen after dose increases.
  • Concerns about whether your treatment plan is still suitable for you.

If any of these rare but more serious symptoms occur, please contact your GP or call NHS 111 immediately:

  • Repeated vomiting
  • Difficulty swallowing
  • Chest pain
  • Signs of dehydration (such as dizziness or very dark urine)
  • Black or tar-like stools
  • Severe upper abdominal pain

Mild to moderate heartburn while taking Wegovy is not automatically a reason to stop treatment, particularly in the early stages when your body is still adjusting. Whether you continue, pause or adjust your dose depends on how severe the symptoms are, how long they last and whether they affect your ability to stay hydrated, eat properly, and feel well overall. In many cases, reflux symptoms improve over time with simple supportive changes.

Instead of stopping treatment on your own, speak with our clinical team here at Phlo Clinic so we can help you decide on the safest and most effective way to manage your symptoms while continuing your treatment plan!
